Here you will find all of our intake forms, policies, & questionnaires. Please submit the forms at least 2 business days before your initial evaluation to allow your therapist time to review them.

New Patients:

Please fill out the New Patient Intake Form, Release Forms, Financial Policies, HIPAA form, Evaluation Policies & Office Policies

Feeding Evaluation:

The following forms are only required if your child is coming for a Feeding Evaluation

Telehealth:

The following forms are only required if your child is participating in Telehealth sessions.
